Start by opening a new file any size you want and write something big in it

PHANTOM

And pick the magic wand tool and go up-under where it says image and click on the add to selection

And then go over the letters

Then go to filter>render>clouds but before you do that make sure that the colors under the tool box are black and white

After using the clouds filter you'll have something that looks like this

Go to filter>distort>glass

Go to layer style>stroke

Last go to layer style> bevel and emboss

And the results should look something like this
